This site does not ask for a medical history, account, payment details or email address. The appointment checklist runs in your browser. Its selections are held in page memory and are not submitted to us. Downloading the selected questions creates a plain-text file on your device.
Reading settings
The larger-text control changes the presentation of the current page and subsequent navigation in the same session. It does not create a profile. Reloading the site returns the control to its default state. We do not set an advertising or analytics cookie through application code.
Hosting and routine logs
The site is hosted through Vercel and uses Cloudflare for domain infrastructure. Infrastructure providers may process routine request data such as IP addresses, requested URLs, user-agent information and timestamps to operate and secure their services. This publication does not ask you to place health information in a URL.
Links to other websites
